Nursing Notes Privacy & Safety
Educational use & privacy
Educational documentation practice only. Use fictional or instructor-provided information. Do not enter real patient information or protected health information (PHI). Documentation requirements vary by organization, setting, role, policy, and applicable law.
Nursing Notes Privacy & SafetyEducational purpose
The Nursing Notes section teaches documentation concepts with guides, fictional examples, blank templates and practice. It is not an electronic health record, a medical record, a patient chart, a clinical documentation system or a medical scribe.
Do not enter real patient information
Use fictional or instructor-provided educational information only. Do not enter names, dates of birth, medical record numbers, contact details, or any other information about a real person. Please do not try to "de-identify" a real note and paste it here; identifiers can remain inside free-text narratives. HHS explains that individually identifiable health information can be protected health information (PHI), and its de-identification guidance lists many kinds of identifiers.

What Dosage Calcs does and does not do
- No EHR functionality, patient lookup, or connection to any health system.
- No uploads of notes, images or documents; no voice recording or microphone.
- No cloud storage of practice-note text. Practice text in the Practice Note Builder stays in the page's memory only and is not intended to be retained. It is not saved to your device, sent to a server, or placed in the web address. Refreshing or closing the page clears it.
- The builder shows a basic reminder if text looks like an email address, phone number or full date. That check cannot find every identifier and does not make the tool suitable for real patient information.
- Dosage Calcs is not designed or offered for protected health information and makes no HIPAA compliance claims.
Examples are fictional
Every example uses fictional information, such as "Fictional Patient A." Examples are not substitutes for your organization's documentation standards, forms or EHR workflows.
Requirements vary
Documentation requirements, charting frequency, correction procedures, incident-report procedures, record retention, scope of practice and state law vary by organization, setting, role and jurisdiction. Follow your school's, instructor's and facility's policies. Nothing here is legal advice.
