How it works

  1. Choose a fictional scenario, or practice freely with made-up details.
  2. Choose a format: narrative, DAR, SOAP, SOAPIE or SBAR. Each field explains what belongs there.
  3. Write your practice note. The scenario already supplies every clinical fact and action; you document what happened.
  4. Then open the example documentation, which uses only the scenario's facts, and compare.

What happens to your text

Your practice text is kept only in this page's memory. It is not saved to your device, not placed in the web address, not sent to Dosage Calcs or any other server, and not used for analytics. Refreshing or leaving the page clears it. There is no upload, microphone, patient lookup or EHR connection. Need a reusable form? Use a blank template.

Do not enter real patient information.

Practice with fictional information only. Do not enter names, dates of birth, medical record numbers, contact information, or any other real patient information.
