What this category covers
Psychosocial Integrity covers supporting the emotional, mental and social well-being of clients and families. The nurse uses therapeutic communication, recognizes coping and stress responses, supports clients through grief and end of life, recognizes signs of abuse, neglect, substance use and suicide risk, keeps clients safe, and reports concerns to the RN or provider.
Key concepts
Therapeutic communication
Open-ended questions, reflection and active listening help clients express feelings. Avoid false reassurance, advice-giving and changing the subject.
Safety first in a crisis
Statements about self-harm or harming others are always taken seriously. Stay with or keep the client under observation per policy, remove hazards, and report immediately.
Recognizing abuse and neglect
Unexplained injuries, fearfulness, or inconsistent explanations need careful documentation and reporting according to law and facility policy.
Grief, loss and end-of-life care
Grief is individual. Support cultural and spiritual preferences and the family's needs.
